What the pipeline guarantees
Evidence or nothing
Every suggested code carries a verbatim quote from the source note. Candidates whose evidence cannot be located in the text are discarded before review.
Context-aware
Negated findings, historical conditions, hypothetical statements and family-history mentions are flagged and suppressed rather than silently coded.
Auditable by design
System extractions and human decisions are recorded as discrete audit events with actor, timestamp and prior value.
